To pre-register as a patient at PureLife Alternative Wellness Center,

you may fax or email us the following items prior to your first visit:

• a copy of a valid California doctor's letter of recommendation
• a copy of your California Driver's License/ID Card
• a completed Registration Form

After having received your paperwork, we will authenticate for validity. You can also call us with information from your recommendation letter so we pre-register/pre-verify you. Upon your first visit to our center you must bring your original doctor's recommendation letter and your California ID/Driver's License.
